Student Work Permits

14–15 Year Old Work Permit

• Section A would be completed by the Minor’s Prospective Employer.
• Section B would then be completed by the Minor’s Parent or Legal Guardian.
• Once Sections A & B have been completed, submit the form to wvvaoffice@westvaacademy.org and Sections C & D will be completed by WVVA and provide copies for the family and the employer and email a copy to the West Virginia Division of Labor (WageandHour@wv.gov) within 4 days of issuance.
• The original is retained by WVVA.

16–17 Year Old Work Permit

• WVVA must receive a written request from the prospective employer identifying their Business Name & Address, a Description of their Business, and a Description of the Minor’s role at the business. This should be sent to wvvaoffice@westvaacademy.org.
• Once the request is received from prospective employer, WVVA will complete the form in it’s entirety and provide copies to the Minor and the Minor’s Employer as well as email a copy to the West Virginia Division of Labor (WageandHour@wv.gov) within 4 days of issuance.
• The original is retained by WVVA.
